Health centers are encouraged to apply to a learning collaborative following the “Acting on SDOH Screening Data to Address Barriers to Health” webinar. This learning collaborative will highlight best practices and facilitate interactions among health center program participants to share strategies, challenges, and successes.

The TB Elimination Alliance will offer up to fifteen mini-grants to organizations to enhance LTBI/TB community engagement and education, provider education, and quality improvement. Additional preference is given to organizations currently serving A/AA and NH/PI populations and interested in expanding to non-US-born Latino American and US-born African American communities.

The Association of Asian Pacific Community Health Organizations (AAPCHO), Health Outreach Partners (HOP), MHP Salud, and the National Health Care for the Homeless Council (NHCHC) will facilitate and lead this learning collaborative. This is an opportunity for health centers serving special and vulnerable populations to explore effective strategies to screen for SDOH and utilize data to build effective practices of addressing SDOH through the provision of outreach and enabling services. 

Join AAPCHO and COVID-19 and Other Infectious Diseases Health Equity Response Network (CHERN) for the first of a four-part learning series designed for clinicians, clinical supervisors, and other clinical staff. The series will cover unique COVID-19 topics, including long COVID/PASC, therapeutics, clinical guidance for vaccines, and promising practices for outreach.
