Research demonstrates correlations between enabling services utilization and improved health outcomes. AAPCHO, HOP, MHP Salud, and NNOHA are co-hosting this webinar to share successful models for diabetes care for special and vulnerable populations served at Federally Qualified Health Centers. The presenters will place emphasis on the health centers’ evolving enabling services or non-clinical workforce, who have a role to play in decreasing the percentage of patients with HbA1c levels greater than 9%.

  1. Understand the roles and responsibilities of the Enabling Services staff and how SDOH impacts diabetes management for S&VP;
  2. Describe 5 roles the Enabling Services staff can play on the health care team for S&VP; and
  3. Introduce tools and resources to address health equity, the relationship to quality, and population health management for S&VP
