The PRAPARE assessment tool consists of a set of national core measures as well as a set of optional measures for community priorities. It was informed by research, the experience of existing social risk assessments, and stakeholder engagement. It aligns with national initiatives prioritizing social determinants (e.g., Healthy People 2020), measures proposed under the next stage of Meaningful Use, clinical coding under ICD-10, and health centers’ Uniform Data System (UDS). PRAPARE emphasizes measures that are actionable. PRAPARE Electronic Health Record templates exist for eClinicalWorks, Cerner, Epic, athenaPractice (formerly GE Centricity), NextGen and more and are freely available to the public as part of our PRAPARE Implementation and Action ToolkitPlease note: This is the latest and finalized version of the PRAPARE tool.

While not mandatory, it is important that users of PRAPARE collect standardized data on ALL of the core measures of PRAPARE.  Standardized data at the organizational, state, and national levels will accelerate population health planning, facilitate benchmarking across organizations, and document patient complexity that can inform payment models and risk adjustment.
