Improving the Health and Well-being of Older Californians and Caregivers

The initial goal of this project is to convene community health centers (CHs) to identify needs, recommendations, and practical solutions to improving the health care and health status of older Californians and caregivers including Asian American (AA), Native Hawaiian and Pacific Islander (NH/PI), and other vulnerable populations served at CHCs. The best practices and recommendations that come out from this convening will support future pilot trainings to enhance the delivery of integrated services that are culturally competent, comprehensive, and coordinated placing older adults and their families at the center of a care team to ensure their care needs are met. Although the current effort is California-focused, AAPCHO intends to expand this effort nationally, particularly in states with the highest percentage of AAs and NH/PIs.
