DESCRIPTION:Join the Association of Asian Pacific Community Health Organizations (AAPCHO) for a Learning Collaborative (LC) to learn more about diabetes and pre-diabetes disparities among the Asian American community. Guest speakers from Joslin Diabetes Center’s Asian American Diabetes Initiative\, Emory University\, University of California\, San Francisco School of Medicine\, and University of California\, Santa Cruz will share in-depth knowledge and effective strategies to support Asian American patients at health centers through non-clinical approaches. Participants will engage in peer learning to share challenges and promising practices. \nThe learning collaborative consists of four 90-minute sessions\, held on Tuesdays in April and May at 7:00am-8:30am HT / 10:00-11:30am PT/ 1:00-2:30pm ET\, with the following topics: \nSection Dates and Topics: \n\nTuesday April 11: Understanding Diabetes Disparities for AA Patients\nTuesday April 18: The Importance of Community and Peer Support\nTuesday May 2: Promoting Mental Health\, Wellness\, and Balance for Patients and Communities\nTuesday\, May 9: Food as Medicine\, Food and Culture\n\nIntended Participants: \nHealth centers\, Primary Care Associations (PCAs)\, Health Center Controlled Networks (HCCN)\, and community organizations interested and/or invested in diabetes prevention and management for AAs. \nFee: No cost/free \nCommitment: Accepted LC applicants are asked to attend a total of four learning sessions between April and May 2023. The webinar will include recommendations to increase access to care\, improve health outcomes\, and promote health equity for Asian American (AA)\, Native Hawaiian\, and Pacific Islander (NH/PI) patients.
