DESCRIPTION:Join the Association of Asian Pacific Community Health Organizations (AAPCHO) for a Learning Collaborative (LC) to learn more about diabetes and pre-diabetes disparities among the Asian American community. Guest speakers from Joslin Diabetes Center’s Asian American Diabetes Initiative\, Emory University\, University of California\, San Francisco School of Medicine\, and University of California\, Santa Cruz will share in-depth knowledge and effective strategies to support Asian American patients at health centers through non-clinical approaches. Participants will engage in peer learning to share challenges and promising practices. \nThe learning collaborative consists of four 90-minute sessions\, held on Tuesdays in April and May at 7:00am-8:30am HT / 10:00-11:30am PT/ 1:00-2:30pm ET\, with the following topics: \nSection Dates and Topics: \nTuesday April 11: Understanding Diabetes Disparities for AA Patients\nTuesday April 18: The Importance of Community and Peer Support\nTuesday May 2: Promoting Mental Health\, Wellness\, and Balance for Patients and Communities\nTuesday\, May 9: Food as Medicine\, Food and Culture \nIntended Participants: \nHealth centers\, Primary Care Associations (PCAs)\, Health Center Controlled Networks (HCCN)\, and community organizations interested and/or invested in diabetes prevention and management for AAs. \nFee: No cost/free \nCommitment: Accepted LC applicants are asked to attend a total of four learning sessions between April and May 2023. DESCRIPTION:What are behind the gaps in latent tuberculosis infection (LTBI)/tuberculosis (TB) prevention and what can health centers and other organizations do to support Asian\, Asian American\, (A/AA)\, Native Hawaiian\, and Pacific Islander (NH/PI) patients with LTBI? \nJoin AAPCHO to gain in-depth knowledge and best practices on how to empower patients at health centers through strategies that effectively address LTBI/TB prevention and quality improvement approaches in A/AA and NH/PI communities. \nThis webinar session will highlight provider and patient resources for clinical monitoring of patients during LTBI treatment. \nPlease note that this public webinar is a part of the 2023 “Quality Improvement Strategies for LTBI Screening\, Testing\, and Treatment in A/AA- and NH/PI- Serving Health Centers: National Clinical Quality Measure Development – Providing Care for Patients with LTBI” Learning Collaborative. \nLearning Objectives: \nUpon completion of this training\, participants will be able to: \n\nExplain the importance of tuberculosis prevention and its contribution to overall goals of TB elimination;\nDescribe easy-to-use tools and other resources to engage and encourage patients during LTBI treatment to improve care cascade outcomes;\nIdentify how to measure the steps in the LTBI care cascade to advocate and reduce preventable losses in the cascade to improve program performance; and\nIdentify strategies to overcome barriers to each step in the LTBI care cascade to successfully implement and improve patient diagnosis and treatment.\n\nSpeaker: \nShereen Katrak\, MD\, MPH\, Medical Officer\, Director\, TB Free California\, Tuberculosis Control Branch\, California Department of Public Health \nIntended Participants: \nHealth centers\, Primary Care Associations (PCAs)\, Health Center Controlled Networks (HCCN)\, and community organizations interested and/or invested in LBTI/TB prevention and management.
