This panel session will discuss how value-based care models are key to addressing SDOH and unmet social needs and promoting health equity. \nSocial Determinants of Health – Medicaid Coverage and Payment\nWednesday\, February 16\n5:00 – 6:16 pm HT / 7:00 – 8:15 pm PT / 10:00 – 11:15 pm ET \nScreening for and addressing social determinants of health (SDOH) are widely recognized as essential for delivering whole person care\, improving health equity\, lowering total costs of care\, and participating in value-based care. Current events underscore the critical importance of addressing SDOH\, with populations served by Health Center Program grantees and look-alikes being disproportionately impacted by COVID-19. The health center model of care uniquely positions health centers to play active roles in integrating SDOH services and aligning with community-based organizations. Furthermore\, health centers are committed to improving health equity. Recent guidance from the Centers for Medicare and Medicaid Services (CMS) provides new clarity on Medicaid and SCHIP authority for reimbursement of SDOH screening and interventions in particular circumstances. Through this session\, NACHC will educate health centers and primary care associations on the opportunities to use PRAPARE and SDOH screening tools to improve the delivery of care through enhanced data collection and relationships with safety-net programs.

DESCRIPTION:The Tuberculosis (TB) Elimination Alliance (TEA) invites you to submit your application to the  TB Learning Collaborative\, “Quality Improvement Strategies for Latent Tuberculosis Infection (LTBI) Screening\, Testing\, and Treatment in Asian\, Asian American (A/AA)\, Native Hawaiian\, and Pacific Islander (NH/PI)-Serving Health Centers: Electronic Health Record (EHR) Template Development”. EHR plays a critical role in optimizing the latent TB infection care cascade\, and the Learning Collaborative will highlight clinical experts in this area. \nThe Learning Collaborative consists of four sessions on Thursdays in February from 10:30am-12:00pm HT / 12:30-2:00pm PT/ 3:30-5:00pm ET.  \nDates: \n\nSession 1 – Thursday\, February 3\, 2022 \nSession 2 – Thursday\, February 10\, 2022\nSession 3 – Thursday\, February 17\, 2022\nSession 4 – Thursday\, February 24\, 2022 \n\nLearning Objectives: \nAfter this webinar\, participants will be able to: \n\nDiscuss challenges and barriers around EHR modification and template development for LTBI screening\, testing\, and treatment;\nIdentify tools to understand and implement EHR templates for LTBI screening\, testing\, and treatment; and \nDevelop a roadmap for EHR template development processes for LTBI screening\, testing\, and treatment.\n\nSpeakers: \nSession 4 \n\nKara Green MSN\, APRN\, FNP-BC\, Director of Clinical Services and Quality Improvement\, HOPE Clinic\n\nCommitment: \nAccepted Learning Collaborative applicants are asked to attend a total of four learning sessions in February 2022. Sessions include interactive activities\, small breakout room discussions\, and presentations by subject matter experts. \nIntended Participants: \nHealth centers\, health center look-alikes\, and other community health organizations interested and/or invested in TB/LTBI screening\, testing\, and treatment. Health centers and health center look-alikes will be prioritized for selection.
