SUMMARY:“The Two Pandemics Impacting Asians and Asian Americans: Anti-Racism & COVID-19” Learning Collaborative
DESCRIPTION:Join the Association of Asian Pacific Community Health Organizations (AAPCHO) to learn about the two pandemics impacting Asian and Asian American (A/AA) community members: anti-Asian racism and COVID-19. This learning collaborative will explore diverse perspectives of the two pandemics and highlight innovative care models for A/AA-serving community health centers.\n\nThe learning collaborative consists of four 90-minute sessions\, held every Tuesday from June 8 through June 29 with the following topics. \n\nSession 1 (June 8): Intersecting Identities of A/AAs\, Native Hawaiians (NHs)*\, and Pacific Islanders (PIs)*\nSession 2 (June 15): Understanding A/AA Community Needs Using a Social Determinants of Health Lens\nSession 3 (June 22): Applying Trauma-Informed Care for A/AA During COVID-19\nSession 4 (June 29): Combatting Two Pandemics with Community Rooted Interventions\n\n\n  \nAfter this learning collaborative\, participants will be able to:\n\n\nBetter understand the diversity of A/AAs\, as well as NHs and PIs*\nExplore social drivers of health that impact A/AA communities\nApply trauma-informed care approaches in serving A/AAs \nDiscuss the two pandemics affecting A/AAs: anti-Asian racism and COVID-19\n\n*This learning collaborative will focus primarily on Asian/Asian American communities that have been disproportionately targeted with violence during the pandemic. AAPCHO recognizes the impact of racism through systemic discrimination that has impacted NHs and PIs before and during the COVID-19 pandemic. Policies that aggregate A/AA\, NH\, and PI population data and limit the eligibility for particular A/AA\, NH\, and PI groups/subgroups to access safety net programs are just two examples that will be further addressed in a future LC. For more information about the effect of COVID-19 on the NHs and PIs\, visit the Pacific Islander Center of Primary Care Excellence’s website: https://pi-copce.org/covid19response/.

SUMMARY:"Incorporating Social Determinants of Health (SDOH) Data into Risk Stratification Models to Improve Health Equity" Webinar
DESCRIPTION:Did you know that 80% of a person’s health is determined by the social determinants of health (SDOH)? Through a process called risk stratification\, health centers can assess SDOH factors to help allocate resources to patients who need them the most.\n\n\nRisk stratification identifies patients with complex health and social needs in order to manage their comprehensive care. By systematically identifying patient and population-level needs\, health centers can provide targeted clinical and non-clinical services to strengthen sustainability and impact for patients’ risk factor data to improve care.\n\n\nThis webinar will cover the foundations of risk stratification through findings from the 2019 Protocol for Responding to and Assessing Patients Assets\, Risks\, Experiences (PRAPARE) Risk Stratification Learning Collaborative. Dr. Milton Armston\, Jr.\, Psy.D.\, the Director of Behavioral Health and Integrative Services will also share risk stratification practices from his work at Community HealthNet Health Centers.\n\n\nLearning Objectives: \n\nAfter this webinar\, participants will:\n\n\nUnderstand the importance of using social risk factor data to improve population health at health centers.\n\nIncorporate takeaways from a national PRAPARE risk stratification model vetted by health centers\, Primary Care Associations (PCAs)\, and Health Center Controlled Networks (HCCNs).\n\n\nLearn from a health center example leveraging social risk data to identify and address individual patient needs.\n\n\n  \n\nIntended Audience: \n\nHealth centers\, PCAs\, HCCNs\, and other health care organizations interested in population health using risk stratification.\n\n\nFee:\n\nNo cost/free\n\n\n\nRegister: \n\nbit.ly/RiskStrWebinar\n\n\n\nA confirmation email will be sent to you after you register\, providing information on how to join the webinar.\n\nIf you have trouble accessing the registration link\, visit bit.ly/RiskStrWebinar or https://zoom.us/webinar/register/WN_t37GeuSXRciPOonJNRKfTg.
