SUMMARY:[Learning Collaborative] “Diabetes Continuum of Care: Environmental Impacts on Diabetes Management and Access to Care” Session 3
DESCRIPTION:Asian American (AA) populations face unique challenges with respect to Type 2 Diabetes. AA populations appear to have one of the lowest rates of diabetes compared to other racial/ethnic groups. However\, when the AA populations are disaggregated\, specific subgroups have significantly higher rates of diabetes compared to other racial/ethnic groups. \nJoin Association of Asian Pacific Community Health Organizations (AAPCHO)\, Farmworker Justice\, and the National Network for Oral Health Access for this Learning Collaborative which will focus on environmental justice and the impacts of a changing climate on diabetes management and access to care. Guest speakers will discuss the intersection of environmental justice with diabetes care and management in health centers\, strategies to support patients impacted by climate-related natural disasters\, and practical steps and tools to support health center staff in promoting access to care. \nThe LC consists of four 60-minute sessions\, held on Tuesdays in March with the following topics: \nTuesday\, March 7\, 10:00-11:00am HT / 12:00-1:00pm PT / 3:00-4:00pm ET \nSession 1: Environmental Justice\, Climate Change\, and Health Center Role \nTuesday\, March 14\, 9:00-10:00am HT / 12:00-1:00pm PT / 3:00-4:00pm ET \nSession 2: Limitations to Access to Care During Climate-Related Natural Disasters \nTuesday\, March 21\, 9:00-10:00am HT / 12:00-1:00pm PT / 3:00-4:00pm ET \nSession 3: Impacts and Effects of Changing Climate on Diabetes Management \nTuesday\, March 28\, 9:00-10:00am HT / 12:00-1:00pm PT / 3:00-4:00pm ET \nSession 4: Practical Steps to Improve Community Health Centers Agility and Response \nLearning Objectives: \nAfter this LC\, participants will be able to: \nDemonstrate the intersection(s) between environmental justice\, climate change\, and diabetes management and access to care; \nDescribe the impacts of climate-related events and natural disasters on diabetes management and access to care; \nIdentify specific impacts of climate-related events and natural disasters on access to diabetes care for agricultural workers\, Asian American\, Native Hawaiian\, Pacific Islander\, and other special and priority health center populations; \nand Share resources and tools for health centers to incorporate climate change considerations into diabetes management and access to care. \nIntended Audience: Community health workers\, patient navigators\, outreach staff and supervisors\, nurses\, clinicians\, care managers\, and other patient-facing health center staff. Applicants from health centers will be prioritized. \nFee: No cost/free \nCommitment Accepted LC applicants are asked to attend a total of four learning sessions in March 2023. SUMMARY:[Webinar] Using Oral Health Equity Data to Improve Patient Care
DESCRIPTION:Despite progress\, disparities in access to dental care and oral health outcomes persist — particularly among underserved populations. One significant challenge: lack of comprehensive and accurate data\, which makes it difficult for oral health providers to deliver culturally specific and linguistically accessible care. \nJoin CareQuest Institute and the National Association of Community Health Centers for a webinar that will provide a fresh look at the state of oral health equity data. Our expert panel will discuss data collection tools and standards\, and then offer solutions on how to turn that information into practical solutions that can meet the needs of specific patient populations. \nLearning Objectives \nAfter the webinar\, participants will be able to: \n\nExplain how oral health equity data gaps impact oral health care for unique patient populations\, including Asian Americans\, Native Hawaiians\, and Pacific Islanders.\nIdentify specific roles and remedies to eliminate oral health equity data gaps.\nRecognize the value of disaggregated data in developing public policies to achieve health equity.\nDiscuss how to use oral health equity data to improve care for unique patient populations.\n\nModerator & Presenter \n\nGrace Wang\, MD\, MPH\, FAAFP\, Senior Fellow Public Health Integration & Innovation\, National Association of Community Health Centers\n\nPresenters \n\nRosy Chang Weir\, PhD\, Director of Research\, Association of Asian Pacific Community Health Organizations\nHuong Le\, DDS\, MA\, Chief Dental Officer\, Asian Health Services
