SUMMARY:[Learning Collaborative] Strategies to Empower Asian American Patients with Diabetes and Pre-Diabetes
DESCRIPTION:Asian American (AA) populations face unique challenges with respect to Type 2 Diabetes. While AA populations appear to have one of the lowest rates of diabetes compared to other racial/ethnic groups\, disaggregated\, specific subgroups have significantly higher rates of diabetes. \nWhat is behind this disparity and what can health centers and other organizations do to support AA patients with pre-diabetes and diabetes?  \nJoin us for a Learning Collaborative (LC) to gain in-depth knowledge about empowering AA patients at health centers through strategies that effectively address diabetes disparities in the AA community. Participants will have an opportunity to engage in peer learning to share challenges and promising practices. Guest speakers are from Joslin Diabetes Center’s Asian American Diabetes Initiative\, Lowell Health Center\, and University of California\, San Francisco School of Medicine. \nLearning Objectives: \nAfter this Learning Collaborative\, participants will be able to: \nUnderstand the factors that influence diabetes disparities for AA patients; \nIdentify promising practices to implement effective diabetes prevention strategies for AA patients; \nIdentify promising practices to implement effective diabetes management strategies for AA patients; \nand Engage in peer learning with participants on strategies and best practices to support AA patients with pre-diabetes and diabetes. \nAs Asian Pacific American Heritage Month approaches in May\, it is important to increase awareness of health disparities in AA and Native Hawaiian/Pacific Islander (NH/PI) populations. DESCRIPTION:The Association for Asian Pacific Community Health Organizations (AAPCHO) is excited to announce the Telehealth Screening Strategies for Improving Whole Person Care Learning Collaborative. This learning collaborative will explore Social Determinants of Health (SDOH) screening strategies in telehealth and highlight innovative care models for virtualizing SDOH screening in community health centers. \nWith the ever-expanding use of telehealth for patient care visits during the COVID-19 pandemic\, health centers must adapt to address the complex health and social needs in a virtual setting. AAPCHO will guide attendees through the development of their own implementation work plan to improve telehealth SDOH screening strategies in their health centers. \nThe Learning Collaborative consists of five sessions on Wednesdays between April 27 through May 25 from 9:00 – 10:30am HT / 11:00am – 12:30pm PT/ 2:00 – 3:30pm ET.

While screening for SDOH is a necessary first step in identifying these disparities\, responding to the data in practice is the ultimate goal in addressing SDOH. \nThe Association of Asian Pacific Community Health Organizations (AAPCHO)\, Health Outreach Partners (HOP)\, MHP Salud\, and the National Health Care for the Homeless Council (NHCHC) will facilitate and lead this learning collaborative. This is an opportunity for health centers serving special and vulnerable populations to explore effective strategies to screen for SDOH and utilize data to build effective practices of addressing SDOH through the provision of outreach and enabling services.  \nThe learning collaborative consists of four 90-minute sessions\, held on Wednesdays in August and September at 8-9:30am HT / 11-12:30pm PT / 1-2:30pm CT / 2-3:30pm ET:  \n\nSession 1: Wednesday\, August 10\nSession 2: Wednesday\, August 24\nSession 3: Wednesday\, September 7\nSession 4: Wednesday\, September 21\n\nLearning Objectives: \nAfter this learning collaborative\, participants will be able to:  \n\nUnderstand the unique considerations of special and vulnerable populations when screening for SDOH; \nIdentify at least 1 strategy to effectively screen for SDOH for special and vulnerable populations;\nIdentify at least three (3) strategies to respond to SDOH screenings and effectively address SDOH of their patient populations;\nDemonstrate the value of screening for SDOH\, providing enabling services\, and responding to patient data; and \nLearn from the cohort’s experiences screening for SDOH and addressing social determinants and will identify best practices for health centers.\n\nEligibility:  \nPriority will be given to applicants who are representatives of a health center program grantee (or potential health center grantee). SUMMARY:[Application Due] Tuberculosis (TB) Elimination Alliance Mini-Grants Program 2022 Request for Funding Proposal (RFP)
DESCRIPTION:The Tuberculosis (TB) Elimination Alliance (TEA) is offering Mini-Grants to organizations serving Asian American (AA)\, Native Hawaiian\, and Pacific Islander (NH/PI) communities that are disproportionately impacted by latent TB infection (LTBI) and/or TB. Additional preference is given to organizations currently serving Asian/Asian American (A/AA) and NH/PI populations with an interest in expanding to non-US born Latino American\, and US born African American communities. \nMini-Grant proposals must align with TEA’s priority areas and activities that reflect community engagement and education\, provider education\, and/or quality improvement. \nTEA Priority Areas: \n\nRaise awareness about the link between LTBI and TB disease\, address misperceptions\, decrease stigma\, and encourage and facilitate testing and treatment for LTBI and TB;\nIncrease awareness of the recommended shorter treatment regimen for LTBI;\nEncourage providers to test and treat LTBI among at-risk populations; and\nCreate a culturally and linguistically appropriate LTBI and TB education\, training and community engagement resources and activities that resonate with high priority AA and NH/PI communities.\n\nUp to 15 awards will be provided to selected organizations. Each award amount will be between $10\,000 and $30\,000. \nDeadline to apply is Friday\, August 5 by 5:00pm local time. \nApplication Timeline and Process: \n\nApplication Submission Period: June 30 – August 5\, 2022\nAward Status Notification: August – September 2022\nAward Contract Process: September 2022\nGrant Period: October 1\, 2022 – June 30\, 2023
