Learn from clinical experts on how EHR plays a critical role in optimizing the latent TB infection care cascade. \nLearning Objectives:\nAfter this learning collaborative\, participants will be able to: \n\nDiscuss challenges and barriers around EHR modification and template development for LTBI screening\, testing\, and treatment;\nIdentify tools to understand and implement EHR templates for LTBI screening\, testing\, and treatment; and \nDevelop a roadmap for EHR template development processes for LTBI screening\, testing\, and treatment.\n\n\n\nIntended Audience: \nHealth centers\, health center look-alikes\, and other community health organizations interested and/or invested in TB/LTBI screening\, testing\, and treatment. AA populations appear to have one of the lowest rates of diabetes compared to other racial/ethnic groups. However\, when the AA populations are disaggregated\, specific subgroups have significantly higher rates of diabetes compared to other racial/ethnic groups. \nJoin Association of Asian Pacific Community Health Organizations (AAPCHO)\, Farmworker Justice\, and the National Network for Oral Health Access for this Learning Collaborative which will focus on environmental justice and the impacts of a changing climate on diabetes management and access to care. DESCRIPTION:The Association of Asian Pacific Community Health Organizations (AAPCHO) cordially invites you to this annual webinar to learn about AAPCHO’s data analysis of the HRSA Bureau of Primary Health Care’s 2021 Uniform Data System (UDS) Data. The webinar will include recommendations to increase access to care\, improve health outcomes\, and promote health equity for Asian American (AA)\, Native Hawaiian\, and Pacific Islander (NH/PI) patients. Presenters will share preliminary findings from AAPCHO’s upcoming report\, “The Health of Asian Americans\, Native Hawaiians\, and Pacific Islanders Served at Health Centers: An Analysis of the 2021 Uniform Data System.” \nLearning Objectives: \nAfter the webinar\, participants will be able to: \n\nPresent an overview of patient demographics\, health utilization\, and patient health outcomes at AA and NH/PI- and NH/PI-serving health centers;\nHighlight key differences between AA and NH/PI- and NH/PI-serving health centers and other community health centers across the nation based on the 2021 Uniform Data System; and\nDiscuss the implications of key trends\, findings\, and opportunities to increase access to care and improve health outcomes at AA and NH/PI- and NH/PI-serving health centers.\n\nIntended Audience: \nHealth centers\, Primary Care Associations (PCAs)\, Health Center Controlled Networks (HCCN)\, National Training and Technical Assistance Partner (NTTAP) organizations\, and community organizations interested and/or invested in health equity for AAs and NH/PIs.
