The health center model of care uniquely positions health centers to play active roles in integrating SDOH services and aligning with community-based organizations. Furthermore\, health centers are committed to improving health equity. Recent guidance from the Centers for Medicare and Medicaid Services (CMS) provides new clarity on Medicaid and SCHIP authority for reimbursement of SDOH screening and interventions in particular circumstances. Through this session\, NACHC will educate health centers and primary care associations on the opportunities to use PRAPARE and SDOH screening tools to improve the delivery of care through enhanced data collection and relationships with safety-net programs.

SUMMARY:[Learning Collaborative] Telehealth Screening Strategies for Improving Whole Person Care
DESCRIPTION:The Association for Asian Pacific Community Health Organizations (AAPCHO) is excited to announce the Telehealth Screening Strategies for Improving Whole Person Care Learning Collaborative. This learning collaborative will explore Social Determinants of Health (SDOH) screening strategies in telehealth and highlight innovative care models for virtualizing SDOH screening in community health centers. \nWith the ever-expanding use of telehealth for patient care visits during the COVID-19 pandemic\, health centers must adapt to address the complex health and social needs in a virtual setting. AAPCHO will guide attendees through the development of their own implementation work plan to improve telehealth SDOH screening strategies in their health centers. \nThe Learning Collaborative consists of five sessions on Wednesdays between April 27 through May 25 from 9:00 – 10:30am HT / 11:00am – 12:30pm PT/ 2:00 – 3:30pm ET.

Let your creativity guide you as we listen to poetry\, music\, and one another’s stories and take time to commemorate heritage month. This workshop is geared towards AA and PI community members but also open to all communities of color and allies. No experience is required!
