SUMMARY:"Incorporating Social Determinants of Health (SDOH) Data into Risk Stratification Models to Improve Health Equity" Webinar
DESCRIPTION:Did you know that 80% of a person’s health is determined by the social determinants of health (SDOH)? Through a process called risk stratification\, health centers can assess SDOH factors to help allocate resources to patients who need them the most.\n\n\nRisk stratification identifies patients with complex health and social needs in order to manage their comprehensive care. By systematically identifying patient and population-level needs\, health centers can provide targeted clinical and non-clinical services to strengthen sustainability and impact for patients’ risk factor data to improve care.\n\n\nThis webinar will cover the foundations of risk stratification through findings from the 2019 Protocol for Responding to and Assessing Patients Assets\, Risks\, Experiences (PRAPARE) Risk Stratification Learning Collaborative. Dr. Milton Armston\, Jr.\, Psy.D.\, the Director of Behavioral Health and Integrative Services will also share risk stratification practices from his work at Community HealthNet Health Centers.\n\n\nLearning Objectives: \n\nAfter this webinar\, participants will:\n\n\nUnderstand the importance of using social risk factor data to improve population health at health centers.\n\nIncorporate takeaways from a national PRAPARE risk stratification model vetted by health centers\, Primary Care Associations (PCAs)\, and Health Center Controlled Networks (HCCNs).\n\n\nLearn from a health center example leveraging social risk data to identify and address individual patient needs.\n\n\n  \n\nIntended Audience: \n\nHealth centers\, PCAs\, HCCNs\, and other health care organizations interested in population health using risk stratification.\n\n\nFee:\n\nNo cost/free\n\n\n\nRegister: \n\nbit.ly/RiskStrWebinar\n\n\n\nA confirmation email will be sent to you after you register\, providing information on how to join the webinar.\n\nIf you have trouble accessing the registration link\, visit bit.ly/RiskStrWebinar or https://zoom.us/webinar/register/WN_t37GeuSXRciPOonJNRKfTg.

SUMMARY:"Strategies to Increase Diabetes Screening Among Asian American (AA) Populations" Learning Collaborative
DESCRIPTION:Asian American (AA) populations face unique challenges with respect to Type 2 Diabetes. When compared to other racial/ethnic groups\, diabetes may not appear to be an alarming concern for AA populations. However\, when the AA data is disaggregated\, specific subgroups have significantly higher rates of diabetes compared to white people and other racial/ethnic groups. Research shows that diabetes screening is a challenge for AA populations that impact this hidden disparity.\n\n\nAAPCHO invites you to join the “Strategies to Increase Diabetes Screening Among AA Populations”  learning collaborative (LC) for an in-depth discussion about diabetes and screening for AA populations. \n\n\nDuring this LC\, clinicians\, health center staff\, and AAPCHO moderators will provide context\, best practices\, and data from a national perspective. Participants will have the opportunity to engage with other health centers from across the United States to share strategies\, challenges\, and successes regarding AA populations and diabetes screening. Health center staff with existing efforts in diabetes screening and prevention are encouraged to apply.\n\n\nThe LC consists of four 90-minute sessions\, held on Mondays in March\, with the following topics: \n\n\nSession 1: Overview of Diabetes Prevalence Among AA Populations\n\n\nMonday\, March 8\, 10-11:30am HT | 12-1:30pm PT | 3-4:40pm ET\n\n\n\n\nSession 2: Diabetes Screening Guidelines for AA Populations\n\n\nMonday\, March 15\,  9-10:30am HT | 12-1:30pm PT | 3-4:40pm ET\n\n\n\n\nSession 3: Implementing Diabetes Screening Protocols for AA Populations at Health Centers\n\n\nMonday\, March 22\, 9-10:30am HT | 12-1:30pm PT | 3-4:40pm ET\n\n\n\n\nSession 4: Direct and Indirect Implications of Screening Protocols\n\n\nMonday\, March 29\, 9-10:30am HT | 12-1:30pm PT | 3-4:40pm ET\n\n\n\n\n\n***Please review session time in your appropriate time zone–daylight savings begins after session 1 and may impact your session times.***\n\n\nAfter this learning collaborative\, participants will be able to:\n \n\n\nIncrease knowledge regarding the health challenges and disparities for AA populations with respect to diabetes and prediabetes.\n\n\nLearn the diabetes screening guidelines as it pertains to AA populations.\n\n\nIdentify appropriate screening protocols for AA patients at their health centers.\n\n\nUnderstand the direct and indirect implications of implementing screening protocols for AA populations.\n\n\n\nIntended Participants: Health centers\, Primary Care Associations (PCAs)\, Health Center Controlled Networks (HCCN)\, and community organizations interested and/or invested in diabetes prevention and management for AAs.\n\n\nFee: No cost/free\n\n\nCommitment: Accepted LC applicants are asked to attend a total of four learning sessions in March 2021.\n
