DESCRIPTION:Research demonstrates correlations between enabling services utilization and improved health outcomes. AAPCHO\, HOP\, MHP Salud\, and NNOHA are co-hosting this webinar to share successful models for diabetes care for special and vulnerable populations served at Federally Qualified Health Centers.
