DESCRIPTION:Did you know that 80% of a person’s health is determined by the social determinants of health (SDOH)? Through a process called risk stratification\, health centers can assess SDOH factors to help allocate resources to patients who need them the most.\n\n\nRisk stratification identifies patients with complex health and social needs in order to manage their comprehensive care. By systematically identifying patient and population-level needs\, health centers can provide targeted clinical and non-clinical services to strengthen sustainability and impact for patients’ risk factor data to improve care.\n\n\nThis webinar will cover the foundations of risk stratification through findings from the 2019 Protocol for Responding to and Assessing Patients Assets\, Risks\, Experiences (PRAPARE) Risk Stratification Learning Collaborative.
