SUMMARY:Diabetes Continuum of Care: Raising the Pillars for Community Engagement Learning Collaborative
DESCRIPTION:Join the Special and Vulnerable Populations Diabetes Task Force for the “Diabetes Continuum of Care: Raising the Pillars for Community Engagement” Learning Collaborative. This learning collaborative will build upon the recent Fall National Learning Series to provide health centers\, Primary Care Associations\, Health Center Controlled Networks\, and other partners with more in-depth information on improving diabetes control for health center patients during the COVID-19 pandemic. \n\nSession dates:\n\nJanuary 28\, 2021\nFebruary 11\, 2021\nFebruary 25\, 2021\nMarch 11\, 2021\n\nAfter this learning collaborative\, participants will be able to:\n\n\n\nDemonstrate key components of successful community engagement to address diabetes amongst special and vulnerable populations\nIdentify ways to adapt and strengthen community engagement strategies to address diabetes during public health emergencies (i.e. COVID-19)\nHighlight tools and resources to assess readiness for fostering community engagement\n\nNTTAP Collaborating Partners: \n\n\nAssociation of Asian Pacific Community Health Organizations (AAPCHO)\nMigrant Clinicians Network (MCN)\nNational Nurse-Led Care Consortium (NNCC)\n\nResources will be available on www.chcdiabetes.org.
